Appointments
Whether you are an existing or a new patient, please call (303) 776-7300. Our staff conducts a simple telephone interview to assure that we are adequately prepared for your appointment. We make every attempt to see patients with urgent medical or surgical conditions as quickly as possibly; usually that day. Alternate care may be provided or suggested in certain cases.
We will attempt to contact you the day before a previously scheduled appointment as a reminder. Click here for directions to our office and alternate means of contacting us.
Administration
Before the appointment we encourage you to take advantage of our on-line forms. By filling out and printing all the necessary information including the Patient Registration and Health History Questionnaire prior to your appointment, we will be able to see you sooner when you arrive.
Payments
All fees not covered by Insurance are due at the time of service. For your convenience, we accept, cash, pre-printed personal checks drawn on local banks, Visa, MasterCard and American Express. Additionally, you may be required to sign a release of medical information and assignment of insurance payment for submittal to your insurance carrier.
Reschedule or Cancellation
If, in the event you are unable to make your scheduled appointment, we ask that you please call us right away. This allows us to better utilize our time with other patients.
